Set Calendar Reminders
Consider setting reminders to reflect on your goals. These reminders could be weekly/bi-weekly/monthly/quarterly, whatever feels sustainable for you. Spacing out your reflection over the year can make your self-evaluation much easier.

Any form/survey tool, like Microsoft Forms, can help you capture accomplishments throughout the year. Then, export the responses and summarize them in your self-evaluation.
